Registration

This document is an agreement between you as a user of Bukit Tinggi Medical Centre (BTMC) Pharmacy Medication Refill Service and BTMC Pharmacy.

Agreement

By using our service, you agree that you have read, understood and are bound by the Terms and Conditions of this service agreement.

You will submit the original prescription and provide any required document(s), information needed and consent to disclosing personal data to BTMC and third-party service providers for the purpose of fulfilling the medication refill service.

Personal data means information such as name, contact number, delivery address, NRIC, personal references, etc. that is required for the provision of this service.

Acceptance

Acceptance of the request is subject to users satisfying the following criteria:

• A valid prescription issued by BTMC
• A valid address for delivery
• Only medications deemed suitable for delivery are accepted (e.g. certain controlled medications under the Dangerous Drug Act and medications requiring refrigeration will not be delivered via courier service)
• Mode of delivery will be delivered via courier service).
• No processing will be carried out on Saturdays, Sundays, and public holiday
• All delivery charges will be borne by the patient and must be settled prior to processing for courier service. Cash on delivery is not available
• Delivery charges are not included in the insurance bill or company GL and must be paid separately by the patient

Additional Terms

Pharmacy reserves the right to approve or reject any request. All decisions are final, and no appeals will be entertained for rejected requests.
